MONTHLY FEE: As stated above. The max monthly family fee will not exceed $300. If paid annually, a 10% discount will be applied. A pro-rated fee will be charged for the 1st month of membership.
ENROLLMENT FEE: A one time non-refundable fee of $100 per member will be charged upon enrollment. The max family enrollment fee is $300.
*Dr. Basil is accepting Medicare patients (which we were previously unable to do due to Medicare laws and regulations). We do not bill insurance for our services and still operate on the membership model. Please call for more information*
